Health Insurance
LBUSD health insurance plans are for Certificated/Classified/Management Employees. For Classified/Management, the provider is through SISC, and for Certificated Employees, it is through CVT. The benefit packages are listed below. The packages can vary depending on which classification you fall under:
Medical with Blue Shield PPO (Classified/Management)
Medical with Anthem Blue Cross PPO (Certificated)
Dental with Delta Dental
Vision with Vision Service Plan (VSP)
Prescription Medication with Navitus
Basic Life 50K paid by the employer for Classified/Management members only
Option for Voluntary life paid by the employee
MDLive (telephone or live chat appointments)
In order to enroll in health benefits we are required to collect the following documents:
Single applicant:
Application filled out
Basic Life Ins. application (Only Classified/Management members)
Voluntary Life Ins. application (optional)
Family applicants:
Application filled out
Basic Life Ins. application (Only Classified/Management members)
Voluntary Life Ins. application (optional)
Marriage Certificate (when adding a spouse)
Taxes 1040 form (when adding a spouse) with financial information blacked out (Classified/Management Only)
Birth certificate for all children under the age of 26 to be added to the plan
*If you plan on keeping a secondary form of medical benefits outside of LBUSD, please provide a copy of your insurance card.
